Surgical removal of naevus pigmentosus can be made of choice, recommended as the only correct one in case of removing moles. It enables not only to get rid of an unwanted/disturbing change, but more importantly enables histopathologic examination of the attained material in order to specify its character properly. It shall be remembered that in case of an early form of melanoma, such a change can look illusively similar to an ordinary mole. While deciding to remove naevus pigmentosus, especially numerous ones, it is recommended to perform dermatoscopy before seeing a surgeon. This examination enables evaluating the morphology of moles and marking the ones whose atopic image is an indication for their surgical removal.
naevus pigmentosus – moles – which have a disturbing dermoscopic image
naevus pigmentosus which are often hurt, prickled
growing naevus pigmentosus of a changing clinical picture (a change of colour, size and shape)
active viral infections (e.g. herpes) and bacterial ones in the area subjected to treatment
in case of taking blood thinning medications ( Polocard, Acenocumarol), it is recommended, if only possible, to phase them out for 5-7 days
using oral form of vitamin A (isotretinoin, acitretin), in view of its ability to impair healing
A surgical treatment of cutting out naevus pigmentosus is performed in topical anaesthesia. A change is cut out with a small piece of healthy skin with the use of a scalpel and then stiches and dressing are put. In order to define the character of the change and to eliminate cancerous process, every removed material is sent to histopathologic examination. It shall also be remembered that in a place of a change removal may remain a more or less visible scar.
Complications: an infection of the treated place, dehiscence of a scar
Convalescence time: 7-10 days
Time: 20-30 minutes
Anaesthesia: yes (local e.g. lignocaine)
Until stiches are removed, it is necessary not to take baths but shower only. After taking a shower the wound must be dried, disinfected with Octanisept and then a dressing must be put. Additionally, in order to make the wound heal better, it is recommended not to make any sudden movements, especially stretching. It is best to resign from any physical exercises in post treatment period. If it is necessary, the patient shall come for a visit to remove stiches in the time stated by a doctor.
